Haim W. was admitted to The Chateau At Brooklyn from another Skilled Nursing Facility after his outcomes did not align with his expectations. He was originally in the hospital due to COVID-19 and, after suffering an embolism and having two surgeries, the doctors in the hospital executed a below the knee amputation. The decision was made by Haim’s Daughter Sima for him to join the family at The Chateau at Brooklyn after l seeing Google reviews shared by previous patients and family members.

When Haim arrived at The Chateau, he was nervous but determined to return home, independently able to take care of himself. Shortly after he arrived, his newly renovated room and warm, friendly interdisciplinary staff which greeted him, relieved his concerns and he refocused on his recovery. The Rehabilitation team worked closely with Haim, creating an individualized treatment plan to assist with building strength in preparation for his prosthetic. Haim met all of his short term and long term goals. Many of these goals were reached through inspiration, mentoring and coaching from Todd Schaffhauser and Dennis Oehler, Paralympic Gold Medalists, who have mentored over 20,000 amputees worldwide. Through CareRite’s signature Amputee Walking School, Todd and Dennis, in conjunction with the Rehabilitation team, oversaw the Comprehensive Amputee Rehabilitation Program (CARP) for Haim. Throughout Haim’s stay, he was pleased with how the staff took care of him. Before he was discharged, Haim shared, “If not for the therapist, I would not have been able to walk again. They really saved me. My physical therapist Rodica Preda was amazing and my occupational therapist Astrud Bryan was phenomenal.” He also mentioned, “Dennis and Todd pushed him throughout his whole stay to really see the light at the end of the tunnel.” It was a beautiful Thursday morning, September 17, 2020, and Haim was up extra early to get home. He was grateful he was able to go home before the Jewish New Year. It was a bittersweet moment for his therapist, staff, Dennis, and Todd to witness his triumphant discharge.

Mrs. Sheila Creasy was admitted to The Chateau at Brooklyn Rehabilitation Center on June 14, 2020, after an inpatient stay at New York Community Hospital. Prior to going to the hospital, Sheila was independent and living on her own. But after she found herself on the floor and spending some time in the hospital she lost her ability to walk and live on her own, which made her worry how she would be able to return back to her apartment after all this. After a long conversation with her siblings, together with the Social Worker, the decision was made that she will be going to a rehabilitation center. After researching local rehabilitation and nursing centers, the reputation of The Chateau led the family to choose The Chateau for Sheila’s rehabilitation needs.

When she arrived at The Chateau, she was greeted by an amazing Interdisciplinary team, made up of individuals from the Nursing, Rehabilitation, Social Work, Activities and Concierge departments. She was very excited to meet her therapist because that was her primary reason for coming here. So, after a good night’s sleep, she was greeted by her Physical and Occupational Therapist. Her therapists discussed Sheila’s expectations and shared she would be able to do all the things she was used to doing. It was a little tough for Mrs. Creasy to visualize meeting these goals. Nevertheless, the therapists assured her she would go home feeling great, and she very motivated to start working with them.

After 2 weeks, Mrs. Creasy started noticing some improvements and she started to do things on her own again. As the days kept going on, she kept feeling stronger and more confident to take another step and push herself a little harder in order to get back to being independent. After a few weeks on an intensive rehabilitation program, Sheila was able to get out of bed on her own and walk 100 ft. with assistance. When asked about her experience with the Rehabilitation team, Mrs. Creasy said “I am so happy with this place, all of the staff are excellent. The therapists constantly kept my goals in front of me and always pushed me to do better than I did the day before.”

On August 12, 2020 Mrs. Sheila Creasy was discharged. She was looking forward to going back home and see her friends and her siblings again.

Mrs. Marie Pierre Louis was admitted to The Chateau at Brooklyn on April 16, 2020, following her inpatient stay at New York Community Hospital. She had been admitted to the hospital with difficulty breathing and fever, where she was diagnosed with COVID-19. She began on a treatment plan including antibiotics and a regiment of medication to treat the virus. With time and attention, Marie began to feel more like herself but felt she was weak and struggling to recover after the long stay in bed on oxygen. As such, she stated she did not feel comfortable returning home alone and decided with her husband and social worker The Chateau at Brooklyn Rehabilitation & Nursing Center would be a wonderful stepping stone to assist her in regaining her independence.

Upon arriving at The Chateau, Marie was exuberant to see the beautiful, semi-private suite she would be staying in. Being an active nurse herself, Marie had inside knowledge of healthcare but wasn’t certain about the changes being made due to the COVID-19 pandemic. On her first day, she was introduced to her physical, occupational, and respiratory therapists, and had an opportunity to discuss her life prior to her hospitalization. She had been completely independent and lived home with her husband. It was her goal to return to her former independence before going home to continue helping others as an active nurse.

As she progressed through her journey, Marie stated that she was very happy with how the nurses and staff were treating her. In her own words, “All of the staff has been exceptionally nice in the way they have treated me, and I feel like I am in a 5-star hotel!” With Marie’s hard work and confidence, paired with the dedication of her treatment team, Marie made great strides of progress towards independence.

By May 11th, 2020, Marie was ready to return home. Her husband greeted her as she walked on her own out of the community, surrounded by supportive staff, signs, balloons, and other congratulatory celebrations. Meeting him with open arms after crushing COVID-19, Marie was a motivational sight to be seen! The Chateau team states shared they were moved by her courage and confidence and wish her and her husband all the best.

Nurse Marie Pierre Louis joined her brave brothers and sisters on the frontlines battling the COVID-19 pandemic when she suddenly fell ill and viewed the pandemic through a different lens: as a patient.

Marie Pierre Louis was admitted to New York Community Hospital in Brooklyn for respiratory distress, later testing positive for COVID-19.

Mr. Kenneth W. arrived at The Chateau on April 5, 2019. Due to the fact that he was a diabetic and also presented with gangrene, he required extensive rehabilitation to gain and build strength. He was scheduled to return to Maimonides Medical Center for a left foot amputation from the knee down. On January 16, 2019, Kenneth was transferred to the hospital to have his left foot amputated.

After the procedure, it was time for Kenneth to return to The Chateau for some much-needed rehab in order for him to learn how to walk again. As soon as Kenneth arrived at The Chateau, he was happy to be placed on the first floor, where he was a resident before the procedure was done. He remembered all the nurses and CNAs on the floor and they were all very happy to welcome him back. When asked about his stay, Kenneth stated “The Chateau is a wonderful place. Ms. Young, my nurse on the 7-3 shift is really amazing, she has a magical touch. When I came in from the hospital, I had some issues that I needed to be taken care of, and within one hour of being here, Ms. Young resolved them.”

After an amazing night’s sleep, Kenneth was greeted by his two favorite therapists, Mr. Melvin from the PT department and Miss. Nagwa, from the OT department. After vigorously working with them together with the world-renowned Dennis Oehler and Todd Schaffhauser from our Amputee Walking School, he was fitted for a prosthetic. He was able to use them on his own and was able to keep up with his activities of daily living. Although all good times must end this was a bittersweet moment when the social worker shared a discharge date with Kenneth, which was a big accomplishment on his part together with his therapist and family members.

It was a beautiful Wednesday morning and Mr. Kenneth was up early, excited but also sad that he was leaving us. The staff was so happy and proud of all that he accomplished over this long five-month stay. The staff at The Chateau at Brooklyn wish Kenneth much success in all that he does in the future, and encourages him to continue pushing and being the best he can be.

Miss. Lynn C. was admitted to The Chateau at Brooklyn Rehabilitation on July 7, 2019. Prior to being admitted to The Chateau, she was admitted to New York Community Hospital, due to the fact that she could not walk. When she arrived at the hospital, they shared with her that her walking challenges might be a result of her two previous strokes and from the fact that she suffered from kidney disease and heart failure. They informed her that the best option would be to enroll in short term rehabilitation for added support. Miss C.’s sister was informed by the social worker about The Chateau at Brooklyn. After she arrived for a tour and selected The Chateau for care services, arrangements were made for her sister to be transferred to The Chateau.

When she arrived at The Chateau, she was really frightened by the whole process and how she would be able to take care of herself. She was doubting that she would be able to walk by herself again. However, shortly after she arrived and saw her beautiful room, as well as all the staff, visiting to greet her, she was relieved and felt a sense of support and comfort.

Throughout her stay, Lynn was pleased with our staff and how they took care of her. Before she was discharged, she spoke with the staff to let them know how grateful she was for everything that they did. “If not for the therapists, I would not have been able to walk again. My physical therapist, Dee Gonzalez, was amazing and my occupational therapist, Sophia Chan, was phenomenal.”

After a few weeks of participating in rigorous rehabilitation therapy, Lynn was able to transfer out of bed on her own, and slowly but surely, she was improving with her activities of daily living and performing self-care tasks independently. After graduating from her wheelchair, and toward her rollator, she was very excited to now be walking on her own.

On September 9, 2019, Lynn was discharged home! Upon discharge, she was happy to be able to walk on her own with her rollator; and she was also able to complete all of her activities of daily living, independently. The team at The Chateau is happy to have had the privilege to be a part of Miss. C.’s healing journey and we wish her nothing but the best at home along with her family.

Details of experience:
Mr. Santo Cuccia was admitted to The Chateau At Brooklyn Rehabilitation Center on January 9, 2019. Prior to being admitted to the Chateau, he was admitted to Methodist hospital due to the fact that he tripped and fell at home. When he arrived at the emergency room, they discovered that he broke his hip and they informed him that they thought surgery would be the best option for him to recover fully and properly. After surgery, Mr. Cuccia was in recovery for months. His recovery was delayed due to episodes of vision loss. During his initial rehabilitation in a different facility, Mr. Cuccia and his wife were unhappy with the services offered and his progress. They believed they could find a rehab center where he could have a better rehabilitation experience. He spoke with a family friend who recommended The Chateau. Soon after, they made arrangements for him to transfer to The Chateau.

Upon arriving at The Chateau, Santo was very happy to see how nice the building looked and how pleasant the general environment was. After staying in the hospital and being bothered by the noise around the clock he was so happy to come to a quiet and peaceful place where he was able to regain his thoughts and think about what was ahead for him. The staff were very kind and welcoming and explained to him the process that they would use to ensure that he achieved his maximum rehabilitation outcome. Although there were some other clinical challenges, starting from the first day he arrived he was determined and excited to start this path to recovery.

Throughout this whole process, Mr. Cuccia was extremely happy with the staff at The Chateau, he was extremely happy with his physical therapist, Parita, and with Niagwa, his occupational therapist. Something that really amazed him was that when he got discharged from the hospital his doctors told him that he would probably have to be on oxygen for the rest of his life, but after working with our respiratory therapist, Arietta, he is now safely off of the oxygen! Santo also shared, “you guys have an excellent therapy team and the most amazing staff!”

Mr. Santo will be going home within one week and we wish him all the best and many more years of continuing to play with his grandchildren and always being there for his family and friends. Here at the Chateau we will really miss having him around but we are comforted by the fact that he is home and healthy enjoying with his children, grandchildren, family, and friends.

Mr. Aaron H. arrived at The Chateau on a cold November day after a short stay at Cornell Medical Center. Prior to going into the hospital, Aaron was a very independent man, all he needed was a walker to get around. However, one morning on his way downstairs for breakfast, he missed a step on the stairs, fell and sustained a pelvic and cervical spinal fracture. He was rushed to the hospital and the doctors performed surgery to put several screws to hold the bones together. After a few days in the hospital, Arthur was feeling a bit better; the doctor informed him that he would be ready to go to a rehabilitation center soon. Since Aaron had been a resident at The Chateau and was very happy with his previous stay, he decided to come back again to get back to his independent self.

Upon arrival at The Chateau, Aaron was warmly greeted by all the staff that recognized him and they were all sad to see the state that he was in after the fall; however, the team gave him strength and reinforcement by explaining to him that he would recover fully, and will return home to his wife. Although he was in a neck brace and had his leg in a cast, he continuously kept a positive outlook. He was always looking forward to working with the rehabilitation team.

When he began working with his therapist, Tracey and Janice, he was unable to bear weight on his legs. The therapists instructed him to perform specific exercises to be able to strengthen his muscles so that he would be ready to walk again once he got the go ahead. After a few weeks of muscle exercises, Aaron was given the go-ahead to do full workouts in the gym and was soon up and walking with his therapists on a walker in the gym.

As the weeks went on, he kept pushing himself and was slowly taken out of his wheelchair and given a walker. Once he got to a point where he was ambulating on the unit with a rolling walker, the staff started prepping Aaron for his discharge.

The staff helped arrange for him to have a stair climber installed on his staircase at home so that he doesn’t have to worry about climbing the stairs. He was also set up with a home attendant to help him and his wife with chores around the house so that they wouldn’t be alone. We want to ensure that our patients are being discharged in the safest manner.

When the time came for Aaron to go home, he expressed to the staff how grateful he was for all that they did for him throughout his stay and told them that he would be recommending to The Chateau to anyone that is in need of rehabilitation.

We at The Chateau are happy to see him going home on his own two feet!
