Mr. Ralph T. was admitted to The Chateau at Brooklyn Rehabilitation Center after taking a fall at home and breaking his ankle. It was a bad break and the doctors recommended that he have surgery to give him a better chance of fully recovering. Prior to falling, Ralph was living at home with his brother and was able to take care of all his needs independently. He was hoping to get back to his old self and was told that a few weeks in rehabilitation would help. Since he was previously a resident at The Chateau, he decided to come back here and work together with the staff that he knew and loved to start walking again.

Ralph was admitted to The Chateau on a Friday afternoon and was happy to be greeted by the friendly staff members. While being evaluated by the therapists, Ralph was unable to put any weight on his ankle and couldn’t walk. He was also unable to take care of any of his activities of daily living such as getting dressed without assistance. The therapists explained to Ralph that if he followed their rigorous plan for therapy, he would be able to leave fully independent. Ralph became concerned about walking up the many stairs he had in his home. It became our goal to erase this concern.

Within a few weeks Ralph was able to start seeing major improvements in his condition. Although he was still unable to walk on his own just yet, he was now able to take care of his ADLs on his own thanks to his Occupational Therapist Genie, and started to see the light at the end of the tunnel. Working together with his Physical Therapist, Dee, Ralph gained the encouragement he needed to keep pushing himself a little further each day. As soon as the Orthopedic Surgeon took off the cast, Ralph and Dee went into overdrive to be able to start walking on his own. As each day progressed, he started seeing more and more improvement and was getting very excited to be able to go back home to his family. By the time Ralph was going home he was independent, he was able to walk on his own and had no problem managing steps which made him very happy.

Prior to being discharged Ralph sat down with the Director of Concierge Services to express his gratitude for all that was done for him while he was at The Chateau.

“This has not been my first stay at a rehab center, but it has definitely been my most positive experience. When I came in here I was very down and sad, I had broken my ankle and was scared that my life would never be the same. From the minute I was brought in the staff made me so comfortable, they took care of all my needs and did it with sensitivity. My therapists were amazing and the nursing staff on the floors went above and beyond to make sure my every need was taken care of. The recreation team is great and was able to keep me involved in activities so that I shouldn’t be bored. I was especially surprised to have a Super Bowl party with some of my friends here, which really made me feel like I was at home. I will be forever grateful to all of the amazing men and women of The Chateau”.

On March 23, Ralph was discharged back home to be able to spend time with his wonderful family. The staff at The Chateau were amazed to see how well he had done while on therapy, and how far he had come from not being able to walk when he came in and then now being able to walk out the front door as if nothing had ever happened. Everyone wished Ralph the best and told him that he must come back to visit every once in a while to bring back his warm smile to our community, a promise which we hope he will fulfill.

Ms. Jessica Eastmond-Rodriguez was admitted to The Chateau in November of 2017 after experiencing a fall at home and a resulting admission to the hospital. Although a young woman, she was experiencing many orthopedic challenges and subsequent pain. When she arrived at The Chateau she was unable to walk and needed assistance with all of her activities of daily living, causing a lot of frustration. She wanted very much to push and continue walking, but every time she attempted to go to therapy, the pain was too much to handle.

After a lengthy discussion with her doctor and therapists at The Chateau, Jessica decided that it best to undergo a knee and hip replacement. The clinicians here at The Chateau recommended that these two procedures would minimize pain and provide her with a great opportunity for successful therapy. She decided to have her hip replacement done first; however, she needed to work with the therapists at The Chateau to build up muscle strength for her new hip. After spending a few weeks working with the therapists and fighting through the pain, the doctors felt she was ready for the hip replacement. Prior to going out for her surgery the staff informed her that as soon as she’s ready to come back from the hospital they were looking forward to helping her continue her progress.

Thankfully, the surgery went well and within a few days Jessica was back at The Chateau and ready to continue on her road to recovery. Within the first few days of returning she was already seeing major improvement and a major decrease in her pain. She was excited to go to the gym every day and get comfortable walking on her new hip. Afterwards, the next step was to undergo a knee replacement.

After scheduling an appointment with her orthopedic surgeon, Jessica went to the hospital for her knee replacement and was back at The Chateau for her final set of rehabilitation services. It is currently one week since that procedure was done, and she is coming along very well. The recovery process will take a few more weeks after which Jessica is hoping to move down to Florida to be with her daughter. Jessica is very happy to be at this point in her progress and thankful for all that has been done for her at The Chateau. She mentioned how all the staff have been great to her and were always willing to go above and beyond to make sure she was comfortable. She was especially appreciative for her therapists, Parita, Marie and Lyuenda for gently and patiently helping her through every step of the process.

We at The Chateau are happy to have been able to help Jessica on her extensive and intricate journey of healing, and are thrilled that she will be going home in a condition that she will be independent and pain free. We wish her nothing but the best and continued success for all her future endeavors.

Annette Young, Professional of the Quarter!

We are proud to share that Annette Young, Licensed Practical Nurse of The Chateau at Brooklyn Rehabilitation and Nursing Center, has been chosen as the third 2017-2018 Professional of the Quarter!

Annette Young has been working as an LPN for fourteen years, but she originally began her career in accounting before transitioning to healthcare. She shared, “I was tired of crunching numbers. I love to help people, I’m a people person. It’s a joy to take care of patients.”

Young is looked at as a major resource in the community, and is happy to be a part of The Chateau team. She shared, “I’m a motivator, for staff and patients. I tell my patients that you’re here to get better, and go home. It’s most rewarding to see someone come in sick and leave better; it makes my day.”

One of her team members shared, “Ms. Young truly exemplifies what it means to be a sincere, caring nurse. Her expertise is extraordinary, and she always patiently explains and cares for her residents as if they were her own family members.”

When she learned of her nomination for Professional of the Year, Young was in shock, stating, “It was so awesome, I was blind-sided! It gives a sense that you’re doing something well, and you’re appreciated. We all work together; The Chateau is tight-knit, and feels like family.”

Young is the third of four professional of the quarters, who will then be in the running for the 2018 CareRite Centers Professional of the Year.

Beloved Resident Turns 103 Years Young

The Chateau at Brooklyn community is thrilled and honored to further share with the community that Mary Todisco has recently turned 103 years young! As a resident of The Chateau at Brooklyn, Mary shared her secret to longevity, “I watch my weight — no sugar, honey,” she said. “If (life’s) not good today, forget about it. Tomorrow, it will be better. Look at it that way. Let it go by. Keep that mind going.”

Ms. Myrna De Peaza arrived at The Chateau at Brooklyn on November 14, 2017 from Mt. Sinai Hospital Kings Highway Division. Prior to being admitted to the hospital, Myrna experienced a fall at home and broke her pelvis. She underwent surgery and was hopeful to make a full recovery; however, seeing as she was unable to put any weight on her hips, she was not capable of going home to live independently as she would have liked to. After discussions with her friends, it was decided that the best option was for Myrna to go to a rehab center and get therapy to help her start walking again, while having the help she would need until she was able to do that. Her family members recommended coming to The Chateau since they were familiar with community from when they had other family members here.

When Myrna arrived at The Chateau she was anxious to start walking again, she was however a little nervous thinking about how much time it would take for that to happen. The therapists explained the process in which they would first try to get her strong enough to get out of bed, while building the muscle strength to be able to put weight on her legs. The nursing staff did a great job helping her with her activities of daily living and reassured her that in due time she would be able to do everything by herself. After a few weeks on therapy Myrna progressed first to be able to get into a wheelchair and then out of the chair and on to a walker. She was thrilled to be seeing improvement with her physical status and was looking forward to be going home in time for the holidays. Although she arrived unable to get out of bed she was slowly seeing improvement in her abilities to get around and even started taking a few steps, this made her very happy.

Myrna continued on this amazing road to recovery until she was able to walk 150 ft. with a walker. This gave her the encouragement that she needed to see that she would be able to go home without worrying about having another accident. Prior to her discharge, Myrna sat down with staff to express her gratitude for all that they have done to help her. “My stay at The Chateau was great. I would like to give a special thanks to my physical and occupational therapists, Tracy and Luanda, who got me back on my feet. The nursing staff on the 2nd floor Ms. Wanda, Ms. Wendy and Mr. Martin, you are all amazing, thank you for making my stay a happy one I will miss you guys. My CNAs Ms. Perrine and Ms. June are the best,
and to Mr. Jacob the Concierge, it was a pleasure having met you. Your smiling face always made me feel better. I must say that I will recommend the Chateau to anyone in need of rehabilitation.”

On December 21st, Myrna was discharged to the community just in time for the holidays. She was very happy to be walking again and to be spending the holidays with friends. She looks forward to traveling during the winter to visit her family in Trinidad. The staff at The Chateau had mixed emotions seeing her off, they were happy for her that she was able to be living independently again, but they were sad seeing a good friend leave. We wish Myrna only good things in the future and hope she comes back to visit us sometime.

Details of Experience:

Mr. Herbert “Herbie” L. was admitted to The Chateau at Brooklyn Rehabilitation and Nursing Center on November 17th from Maimonides Medical Center. Herbie admitted to the hospital after experiencing a fall at home and subsequently breaking his wrist. Prior to going to the hospital, he was living at home with his wife and taking care of his activities of daily living without any assistance. In addition to the broken wrist, Herbie was also experiencing difficulty walking. Going home from the hospital with a cast would have been very hard for Herbie, so he decided to first visit a rehab center where he would be able to learn how to take care of himself and strengthen his legs a little along the way.

Since his wife had previously been an overly satisfied patient of The Chateau, and that he lives locally, the decision was made for him to come to The Chateau as well. When he arrived at The Chateau he was warmly welcomed by all the disciplines, who each explained their roles, and how they would be helping him in his healing process. At first he was unable to walk without assistance and needed help getting out of his chair. He was also incapable of using his hand properly, which made him feel a little down. Spending time up in the gym helped Herbie both physically and mentally. His therapist, Brandon, showed him the proper technique to stand up out of his chair, and gave him the power to believe in himself, by seeing that if he focused on it he has the power to succeed. After spending a couple of weeks in therapy, he is now able to return home with the ability to walk with a cane and can perform his activities of daily living with minimum assistance.

On Wednesday, November 29th, Herbert was happy to return home to his family in time for the upcoming holiday season. He was thrilled to share with all the staff how much gratitude he had to them, and the help he received while rehabilitating at The Chateau. The staff all wished him well and a happy holiday season, and hopes that he continues to enjoy the time he spends with his friends and family.

Mr. Ron G. was admitted to The Chateau at Brooklyn on September 28th 2017 after an inpatient stay at Coney Island Hospital. Prior to being admitted to the hospital, Ron’s wife noticed that he was not acting himself, which made her nervous and she decided to bring him to the emergency room. What they found there was that Ron had contracted a UTI, which can cause a person to become confused and disoriented. After being admitted to the hospital, Ron was started on a regimen of medication to treat the infection. Ron thankfully began to heal; however, after spending so much time in bed without movement, his legs weakened and thus he was unable to walk on his own or complete his activities of daily living. After much discussion with his wife, they had made the decision to seek rehabilitation. Living quite closely to The Chateau and the community’s outstanding reviews, they decided to admit to the center.

Upon arriving to The Chateau, Ron was excited to see the beautiful semi private suite that he was in. He was however concerned being that his primary language was Russian how he would communicate with the nurses and other staff members at The Chateau. Ron was told not to worry as we have plenty of Russian speaking staff, within a few minutes it was arranged to provide Ron with a Russian speaking nurse, who completed the initial evaluation without any obstacles or confusion. Ron explained to the nurse how prior to his hospitalization, he was living a totally independent life along with his wife at home; he was hoping to return to that state before returning home. Ms. Almaz, his nurse, explained the procedure to him of how the process was going to work at The Chateau, and it was arranged that the following morning a Russian speaking therapist would meet Ron to help him with his therapy.

The therapists who with Ron the following morning explained to him that with a good amount of participation and a little patience, they would have him up walking and even climbing steps so that he would be able to spend time at the adult day center where all of his friends spend a great deal of time. Along the journey Ron was very happy with the progress in therapy and he was especially happy with how well the nurses and all other staff members were treating him. He was quoted saying “All of the staff have been exceptionally nice in the way they have treated me, I know that it can sometimes be challenging to deal with, a resident who has a bit of a language barrier. But any time I would have an issue they would always manage to somehow or another solve it in a timely manner. The therapists here do a great job motivating the residents to get out of bed and to the gym to give them the therapy they need, even though some days it was a bigger challenge than others I never once regretted the sessions at the gym and am happy to be going home in the state that I am in now.”

On November 2nd, Ron was very excited to be going home. His friends from the senior center arrived to visit and greet him, and were very excited to see how much he had progressed in just a few, short weeks at The Chateau. They were all thankful to get their friend back as he was also happy to be able to rejoin them at the center. We at The Chateau wish Ron continued success in his future and hope he and his wife have only good times to share in their future.

Details of Experience:

Mrs. Marjorie Feldheim was admitted to The Chateau at Brooklyn Rehabilitation and Nursing Center on July 7th, 2017, subsequent to an inpatient stay at Mt. Sinai Hospital Kings Highway Division. Prior to admitting to the hospital, Marjorie was independent and living on her own, but after developing sepsis and spending a great deal of time in the hospital, she lost her ability to independently ambulate. She was uncertain of her potential of returning home due to her current state-of-health. Marjorie agreed with her son and clinical team that it would be in her best interest to seek out therapy at a neighboring rehabilitation center. Marjorie selected The Chateau as close friends of hers were present at the center as short term rehabilitation patients.

Upon arrival, she was greeted by the amazing team of professionals at The Chateau, who introduced and shared their department and how they would each support her recovery journey. Marjorie shared that she was excited to hear about the wide-range of therapeutic activities offered by the recreation department, as well as an eagerness to look forward to each meal. Marjories’ main goal and primary focus for being a patient at The Chateau was the therapy and healing.

Marjorie met Melvin, her physical therapist, and Janice, her occupational therapist, who explained different techniques they would introduce to assist her in meeting her goals. Melvin and Janice asked Marjorie what her personal goals were since discovering she lived alone prior to her hospitalization. With an inspirational outlook, Marjorie shared that she would return to her health and in due time would be able to to do all the things she loved prior to falling ill. Although it seemed difficult to imagine such strides during this time, the team at The Chateau were overly impressed with Marjorie’s positive outlook on healing and were equally motivated to support her in her journey.

In just a few short days, Marjorie began to notice small improvements in her ability to complete tasks on her own. She began to feel stronger and more confident to take another step forward and push herself a little harder in order to get back to being independent. When asked about her experience with working with the therapy team, Marjorie shared “I am so happy with the environment in the gym; all of the staff are superb. The therapists constantly kept my goals in front of me and always pushed me to do better than I did the day before. They are also very kind, even when I couldn’t walk, they never made me feel incompetent, they just motivated me until I reached my goals.” After only a short time with being on an intense rehabilitation program, Marjorie was able to transfer out of bed by herself and walk up to 100 feet with the assistance of only a walker! She was incredible pleased with herself, giving her the confidence to begin planning her discharge back home.

On September 29th, Mrs. Feldheim discharged from The Chateau back to her home. She was excited to be able to spend the Jewish Holidays at home with her family. The Chateau team wished her nothing but the best and are honored for the opportunity to have assisted an incredible woman achieve her goals.

Ms. Maxine Rosenberg was admitted to The Chateau at Brooklyn on May 17, 2017 from Maimonides Medical Center. She was admitted to the hospital after experiencing a fall at home, and was notified that she had fractured her hip and wrist. Prior to her fall, Maxine was a very active and independent woman. She was very upset that she wouldn’t be able to return home and continue life the way she was used to living it. When voicing her concerns to the social worker at the hospital, they suggested that she admit to a rehabilitation facility where she would be able to obtain the assistance she needed to begin walking and utilizing her hand once again. After researching multiple facilities, she selected The Chateau at Brooklyn after receiving strong positive reviews and the proximity to her home.

Upon arriving to The Chateau, Maxine was greeted warmly by all disciplines. Each manager shared with her each of their unique roles to support her road to recovery. During this time, Maxine explained to the team how she was formerly capable to complete all Activities of Daily Living independently, and that she was worried that by being in a wheelchair and having her hand in a cast, she wouldn’t be able to maneuver at all and succeed in therapy.

The team assured her that even though at first it might be a little tough, once she begins the rehabilitation program, she would notice great progress. The therapy team showed her how to adapt to her injuries as her fractures continued to heal, while also providing therapeutic exercises to stimulate movement and range of motion.

At first, when she began in the rehabilitation gymnasium, she was noted as totally dependent and required assistance for all activities. After an intense rehabilitation journey, which included Maxine to participate in therapy six days per week and two hours per day (physical and occupational therapy), Maxine was finally able to return home completely independent and with the ability to perform Activities of Daily Living with total independence!

Prior to return home, Ms. Rosenberg shared her utmost gratitude to The Chateau for all that they have done for her in her time of need. “All of the staff at The Chateau have been really wonderful and helpful. Tracey, my PT, was excellent; she was there for me every step of the way, ensuring I was up on my feet and walking, all while motivating me and giving me the confidence that I needed to get to walk again. Erika, my OT, was also great! She helped me relearn the skills that I needed to use my hand again after it was casted for so long. All the nurses on the unit have been great, including Ms. Young and Ms. Tadros; they are very professional, warm and really caring. The nurses care about how their patients are feeling. The aids that have helped me have all been so gentle and soft while taking care of me. I loved sitting in the lobby at night and chatting with Denise, the Receptionist; she has a great listening ear. Marsha, your Medicaid Coordinator, was very helpful and held my hand throughout the very complicated process of getting all of the nooks and crannies of my insurances worked out. I will be forever grateful for what everyone here has done for me in my time of need”.

On Friday, August 25th Maxine returned to her home accompanied by her lovely friends who assisted her in settling back into her apartment comfortably. She was excited to be going home and to see her birds again, but promised everybody at The Chateau at Brooklyn that she would come back to visit us and say hello every now and again. We at The Chateau wish her nothing but good health and happiness!
