Details of experience:

On July 8th, 2021, Mr. Dexter Jacobs was admitted to The Chateau at Brooklyn Rehabilitation and Nursing Center. After a brief stay at Metropolitan Hospital, Mr. Jacobs was informed he would need to go for some rehabilitation. With his wife’s help, they found the best place to support Mr. Jacob’s journey to better health. Mr. Jacobs and his wife lived together for many years, and his goal was to return home with her. They both came to the agreement that he would need to be at The Chateau for a little while until he was strong enough to go home.

After he came to The Chateau, Mr. Jacobs was seen by the administrator, concierge, and nursing supervisor. The first words that Mr. Jacobs said were, “Wow, this is a 5-star hotel”! He was comfortable being in the community and ready to get to work from that point on. After settling in his room, Mr. Jacobs was seen by the nursing team on the floor and by the therapists who would work with him every day until his discharge. They introduced themselves, performed their evaluations, and developed a plan to get Mr. Jacobs home as quickly and strongly as possible.

When the first therapy session finished, Mr. Jacobs could be seen with a big smile on his face. He was ecstatic to see that he would be working with a great and friendly team who would push and support him and get him stronger. After a few weeks of therapy, he could be seen walking the halls independently with his walker. Prior to coming into The Chateau, Mr. Jacobs could barely even stand, and walking with a walker was a huge step in completing his goal.

With a few more weeks of rehabilitation, Mr. Jacobs felt stronger and ready to go. From that point, the team set a discharge date of October 16th, 2021. Before he walked out of the building with his wife, Mr. Jacobs personally thanked each staff member who contributed to his healing process. He made sure to thank each nurse, aide, housekeeper, dietitian, among other departments, to show his appreciation.
